We are currently seeking a skilled and compassionate Support Coordinator to join our team and help NDIS participants navigate and manage their plans effectively.

Key Responsibilities:

  • Assist NDIS participants in understanding and implementing their NDIS plans.
  • Coordinate a range of supports (both funded and mainstream) to meet the participant’s goals.
  • Build strong relationships with participants, their families, and other service providers.
  • Ensure participants are connected to services that best meet their individual needs.
  • Manage budgets for support packages and maintain accurate records of services delivered.
  • Provide ongoing guidance and advice to participants on how to maximize their funding and reach their goals.
  • Monitor participant progress and adjust supports as necessary.
  • Regularly review plans and work collaboratively with participants to adjust strategies as their needs change.
  • Advocate for participants and ensure their voice is central to all decision-making processes.
  • Complete progress reports and maintain compliance with NDIS guidelines.

About You:

  • Tertiary qualifications in Social Work, Psychology, Disability, Community Services, or related field.
  • Demonstrated experience in a similar role, especially within the NDIS framework.
  • Excellent understanding of the NDIS, including plan management, funded supports, and capacity building.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work independently and manage competing priorities.
  • Problem-solving skills with a solution-focused approach.
  • Experience working with people with disabilities or in a community-based setting.
  • Current NDIS Worker Screening Check and Working with Children Check (if applicable).
  • Valid driver’s license and access to a reliable vehicle.
